Beta-adrenergic blockade in the treatment of exercise-induced paroxysmal ventricular tachycardia.

چکیده
PAROXYSMAL ventricular tachycardia of the type provoked by exertion or excitement is a rare condition well described by Wilson et al. in 1932.1 The circumstances under which paroxysms occur suggest that sympathetic stimuli are important in producing the arrhythmia. Responses to sympathetic stimuli have been classified by Ahlquist as either a-adrenergic or /3-adrenergic.2 Chronotropic and cardiac arrhythmic effects are in the latter group3 and blockade of f3-adrenergic responses is a rational approach to therapy. The /8-adrenergic blocking drug pronethalol (or nethalide) is effective in the treat-

Orthostatic hypotension associated with paroxysmal ventricular tachycardia.
Two patients (aged 46 and 49 years) are presented who gave a history of several years' duration of unsteadiness, dizziness, and syncopal attacks on standing. Both had orthostatic hypotension which was associated with the development of a unifocal paroxysmal ventricular tachycardia. There was no evidence of organic heart disease. In one of the patients the symptoms usually developed when standin...
متن کاملLeft ventricular function in hypothyroidism. Responses to exercise and beta adrenoceptor blockade.
The effects of exercise and beta adrenoceptor blockade on left ventricular function were assessed in eight patients with hypothyroidism before and during thyroxine replacement treatment. Left ventricular ejection fraction, measured by radionuclide ventriculography, was reduced in hypothyroid patients at rest and on exercise. The rise in ejection fraction with exercise was, however, similar in b...
متن کاملCalcium channel blockers and beta-blockers versus beta-blockers alone for preventing exercise-induced arrhythmias in catecholaminergic polymorphic ventricular tachycardia.
BACKGROUND The mainstay of therapy for catecholaminergic polymorphic ventricular tachycardia (CPVT) is maximal doses of beta-blockers. However, although beta-blockers prevent exercise-induced ventricular tachycardia (VT), most patients continue to have ventricular ectopy during exercise, and some studies report high mortality rates despite beta-blockade. OBJECTIVE The purpose of this study wa...
متن کاملFavourable outcome in idiopathic ventricular fibrillation with treatment aimed at prevention of high sympathetic tone and suppression of inducible arrhythmias.
OBJECTIVE In the absence of an obvious cause for cardiac arrest, patients with idiopathic ventricular fibrillation are difficult to manage. A subset of patients has inducible arrhythmias. In others sympathetic excitation plays a role in the onset of the cardiac arrest. This study evaluates a prospective stepped care approach in the management of idiopathic ventricular fibrillation, with therapy...
متن کاملTilt-induced asystole: a useful prognostic marker or clinically unrelevant finding?
vagal or adrenergic profile. Ectopic foci in fact form the arrhythmia trigger, the behaviour of which may be sensitive preferentially to vagal or to adrenergic influences. It is our experience that vagal arrhythmias can be cured by a focal ablation. These foci, however, are more often sensitive to adrenergic than to vagal stimulation. Waxman et al. made some interesting observations some years ...
